Study Reveals Link Between Semen Bacteria and Pregnancy Success

Infertility has long been perceived predominantly as a female issue, leading to an imbalance in the scrutiny of male factors contributing to conception difficulties. While medical professionals assert that the ability to conceive relies on both partners, societal perceptions have frequently sidelined male infertility. Historically, when couples faced challenges in having children, the conversation often centred on women, reflecting deep-seated societal attitudes.

A recent study from Denmark, published in the Lancet Obstetrics, Gynaecology, and Women’s Health Journal, seeks to shift this narrative by examining a crucial aspect of male fertility: the semen microbiome. This investigation involved analysing the microbial populations present in the semen of men from two distinct groups: one undergoing assisted reproductive technology (ART) treatments and another comprising couples experiencing repeated pregnancy losses.

The researchers noted that, while earlier studies largely focused on the microbiomes within the female reproductive organs, the contributions of the male seminal microbiome had been largely overlooked.

In this study, involving 353 women and their male partners, the team employed advanced sequencing techniques to assess microbial strains in semen, gut, and vaginal samples. The results indicated that around 24 per cent of men in the IVF cohort and 41 per cent of men in the miscarriage group exhibited an imbalance in their seminal bacteria, a condition referred to as seminal V-dysbiosis.

The findings revealed a notable correlation between balanced seminal bacteria and successful pregnancy outcomes; specifically, 85 per cent of couples in the IVF group achieved live births when the male partner had a healthy bacterial balance, in stark contrast to just 57 per cent among those with seminal dysbiosis.

The semen microbiome is a collective term for the various microorganisms—primarily bacteria, such as Lactobacillus, Prevotella, and Pseudomonas—found in seminal fluid. Once regarded as a sterile environment, semen has been shown to host a diverse array of microbes that play vital roles in sperm health and male reproductive capabilities. High levels of Lactobacillus are typically associated with improved sperm motility and overall fertility, while an abnormal proliferation of certain bacteria like Prevotella and Ureaplasma is linked to compromised sperm quality.

This research underscores the necessity for a more comprehensive understanding of male contributions to fertility issues. As the scientific community continues to investigate the complex dynamics of microbiomes, emerging evidence may pave the way for novel approaches to fertility treatments and a reevaluation of how the roles of male and female partners are perceived in reproductive health. This holistic view can lead to better-informed clinical practices that account for factors beyond traditional assessments of fertility. In summary, this study marks a significant step in recognising the relevance of the male microbiome in reproductive health and the importance of considering both partners in fertility discussions.
